The transition of care can be hard, but together we can make it a little easier. If you’re between age 18 and 30, join this group of young adults who are considering the transition to adult CHD care. This is a drop-in program offered by Ahmanson/UCLA Adult Congenital Heart Disease Center and ACHA. All are welcome, from anywhere in the country, and you may take part as often as you like.
Join us as we support each other and navigate moving from pediatric to adult heart care. Bring your experience and listening skills and share as much or as little as you like about the process. A volunteer from ACHA who has been through the transition of care will be on hand to participate, support you, and connect you with resources.
