ACHA Personal Health Passport
Designed to be carried in a pocket or purse, ACHA's Personal Health Passport is a mini-booklet, with removable emergency wallet card, offering congenital heart defect (CHD) patients an educational and patient-friendly format in which to store key heart health information. In addition to sections for diagnoses and medical history, the Personal Health Passport highlights special issues for CHD patients such as exercise recommendations and medical "red flags." Whether you are an adult congenital heart defect (ACHD) patient wanting easy access to emergency health information or a health care provider looking for ACHD health education tools, we encourage you to use an ACHA Personal Health Passport.
